Incident Reporting
It is important that members are aware of the need, and their right, to report incidents that may occur in the course of their working day.
These incidents could be the result of verbal or physical abuse from a pupil, an accident occurring from using equipment or undertaking a task or anything else that results in an injury or near miss.
Members should use the Stirling Council reporting system, ASSURE, to complete incident reporting forms. This can be found here.
Although users do not have to be, we would advise members to ensure they are logged into the system before completing the report. This will allow members completing te report to retain a copy of what was submitted and be notified on it's progress.
When completing the form, members are also encouraged to select NO when at the Trade Union section:-
The default is set to YES, and although this may seem the most appropriate answer, the wording of the statement makes ther NO option more appropriate. Selecting NO allows the appropriate information to be shared with yout TU Health and Safety Rep and makes supporting members in cases far easier. Members should note however the TU H+S reps are not automatically notified of the uincident - members with serious concerns should contact Daniel Auldjo as their lead Stirling EIS Health and Safety Rep at auldjod07s@glow.sch.uk.
Members should also request from their line manager a copy of the corrective measures entered into the form by their line manager or the person undertaking the investigation.
